What Chronic is

Chronic connects you with independent, licensed clinicians for the ongoing management of chronic conditions. Chronic is not an emergency service and is not a substitute for emergency care. If you are in crisis, contact emergency services (UAE 999, India 112).

Clinical decisions - including whether any medication is appropriate for you - are made by the treating clinician after review. Nothing in the app is a promise of a particular outcome.

Your account

You must be 18 or older and give accurate information about your health. Your account is yours alone; do not share your sign-in. You may close your account at any time by writing to support@chronic.md.

Chronic Care Plan subscription

The Chronic Care Plan is an auto-renewing monthly subscription that includes messaging with your care team, video consultations, body-composition and lab tracking, and your monthly Wins report. Laboratory work, medication and any in-person care are charged separately and are not part of the subscription.

• Length of subscription: one month, renewing monthly.
• Price: the price shown in the app before you confirm, in your local currency. An introductory price, where offered, is shown on the same screen and applies only to the number of periods stated there.
• Payment is charged to your Apple Account at confirmation of purchase.
• Renewal: the subscription renews automatically unless auto-renew is turned off at least 24 hours before the end of the current period. Your account is charged for renewal within 24 hours before the end of the current period.
• Managing and cancelling: you can manage the subscription and turn off auto-renew in your Apple Account settings after purchase. Cancelling stops future renewals; the current period is not cancelled or refunded.
• Refunds for App Store purchases are handled by Apple under Apple's policies.

Acceptable use

Do not misuse the service: no unlawful use, no attempts to obtain medication outside a clinician's assessment, no interference with the service or other people's data.

Changes and contact
